About Vineyard Park Surprise
Vineyard Park Surprise is an assisted living center at 16650 North Stadium Way in Surprise, licensed by ADHS as VINEYARD PARK OF SURPRISE with a capacity of 142 residents at the directed care level, the highest of the three ADHS assisted living care levels. State records also list the facility as approved for personal care and supervisory care services. The community operated for years as Mariposa Point of Surprise; a 2022 news report noted its sale from the McFarlin Group, on a 3.88 acre lot. In April 2026, the company, previously known as CarePartners Senior Living, rebranded its portfolio of 34 communities under the name Vineyard Park Senior Living, headquartered in Mercer Island, WA. The community's website describes Vineyard Park Surprise as offering memory care support, medication management, and mobile visits from primary care, podiatry, dental, vision, psychiatric and therapy providers, with daily medication delivery handled by Salibas Pharmacy. Residents live in private studio, one-bedroom or two-bedroom apartments around a courtyard with a fountain, and the company lists activities such as chair sports, weekly Walmart trips and holiday light tours. Respite stays of a week to several months are offered, along with a program intended to let qualifying couples continue living together as care needs change.

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
