Licensed social work support, at no cost to families

Integrity Senior Placement includes a licensed social worker (LMSW) on our care team. Beyond finding the right community, she helps families understand and access the benefits they may already qualify for, and there is never a charge for this support.

More than placement: a social worker on your side

Many families are entitled to help they never claim, simply because nobody walked them through it. Our social worker is a Licensed Master Social Worker, licensed by the Arizona Board of Behavioral Health Examiners, and she works alongside our placement team on the parts of a care transition that go beyond choosing a community: benefits, coordination between providers, and the emotional weight of the change itself.

Every family's situation is different. These are the areas where she most often makes the difference.

Reviewing eligibility for ALTCS

She reviews your loved one's medical and financial picture against the requirements of ALTCS (Arizona Long Term Care System), so you know whether Arizona's Medicaid long-term care program may help pay for care before you apply.

Understanding VA benefits, including Aid and Attendance

She helps veteran families understand the VA benefits that can offset the cost of care, including the Aid and Attendance pension, and connects you with an accredited VA benefits resource when it is time to file.

Coordinating Medicare, hospice, and home health

When a hospital discharge, a hospice decision, or a home health order is in motion, she helps you understand how the pieces fit together and coordinates with those services so nothing falls through the cracks.

Connecting you to community resources

From respite care to caregiver support groups to local senior programs, she connects your family with community resources and caregiver support that lighten the load.

Psychosocial assessment

She completes a psychosocial assessment of your loved one's medical needs, daily functioning, and social well-being, so the level of care is matched accurately the first time and a second move is far less likely.

Support through the emotional side

A care transition is hard on the whole family. She listens, answers questions honestly, and supports you through the emotional side of the move, not just the logistics.

There is never a charge to families for this support

Like all of our placement services, social work support is free to your family. We are compensated by the communities we partner with, the price we negotiate does not affect what you pay, and Arizona requires us to give every client a disclosure explaining how we are paid.

Common questions about our social work support

What is an LMSW?
LMSW stands for Licensed Master Social Worker: a social worker who holds a master's degree in social work and is licensed by the Arizona Board of Behavioral Health Examiners. Our social worker brings that training to every family we serve, from benefits navigation to psychosocial assessment.
Does the social worker's help really cost nothing?
Yes. There is never a charge to families for this support, just like the rest of our placement services. We are compensated by the communities we partner with, and the price we negotiate does not affect what you pay. Arizona requires us to give every client a disclosure explaining how we are paid.
Can she help us qualify for ALTCS?
She reviews eligibility with you and helps you understand the process, the assessment, and the paperwork. Final eligibility is decided by the state through AHCCCS, and for complex financial situations we also connect you with a benefits counselor or elder-law attorney. You can verify current figures at azahcccs.gov.
Can she file a VA claim for us?
No. VA claims must be prepared and filed by an accredited agent or Veterans Service Officer, and we respect that boundary. She helps you understand whether Aid and Attendance may apply to your family, then connects you with an accredited VA benefits resource who can handle the claim itself.
What is a psychosocial assessment?
A structured look at your loved one's medical needs, daily functioning, and social and emotional well-being. It is how we match the level of care accurately the first time, so your loved one is far less likely to face a second move because the first community was the wrong fit.
