About Belmont Senior Living
Belmont Senior Living is licensed under the name BELMONT SENIOR LIVING LLC, as an Assisted Living Home-Directed. Directed care is the highest of the three levels ADHS sets for assisted living, and AZ Care Check lists directed, personal and supervisory care services here.
According to the home's website, every resident gets an individualized care plan, and the help runs from medication management and administration to incontinence care and help with dressing, toileting, mobility and transfers, with memory care support for residents with dementia or Alzheimer's. The site also lists hospice care coordination, palliative care support and end-of-life care, and says the goal is for residents to stay with the home for the remainder of their lives.
The website says meals are part of the care package, with home-cooked meals and snacks and special diets accommodated. Visiting is open-door, and guests are asked to wrap up by 8:00 p.m. unless special arrangements are made.
Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.

Care and services
ADHS lists the home at the directed care level, which the state defines as care for residents who cannot direct their own care. The company's website goes back over the everyday help (individualized care plans, medication management and administration, incontinence care, and support with dressing, toileting, mobility and transfers, plus memory care support for dementia or Alzheimer's) and adds 24-hour care and supervision and a low staff-to-resident ratio. It also lists coordination with physicians, home health agencies and other healthcare providers, along with hospice care coordination, palliative care support and end-of-life care.
Daily life
Beyond care, the website lists exercise and movement programs, activities for mental, emotional and cognitive stimulation, and social engagement and companionship. Laundry and housekeeping are on its list of services, along with a bed turned down each day.

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
