Arizona state record
What the inspections show
In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).
The state cited eight deficiencies at its last routine inspection (October 17, 2025, a visit that also looked into a complaint). 55% of similar communities had 1 or fewer at theirs.
15 deficiencies cited at the routine inspection before it, in 2024.
Eight complaint inspections in the past two years led to a citation, and four others found nothing to cite. 50% of similar communities had 2 or fewer.
ADHS took four enforcement actions in the past two years, with civil penalties of $8,000 in all. 33% of similar communities had none.
Cited at more than one inspection since December 2023: Service plans (seven inspections), Staffing and staff records (seven inspections), Administration (six inspections), and 10 other areas.
ADHS accepted its plan of correction for 58 of the 61 deficiencies cited in the past two years. Care Check lists the rest as: POC Rejected (3).

What was cited on April 8, 2026, in plain words
Administration
The duties of the owner and manager: written policies, a caregiver in charge on site at all times, background checks, and telling ADHS about changes such as a new manager.
It is about who is accountable for care at any hour and whether the home runs by its own written rules.
Cited at 50% of similar communities in the past two years.
Ask on a tour Who is in charge when the manager is not here, and how do I reach them?
Staffing and staff records
Caregiver qualifications and records: skills checked before working alone, training, CPR and first aid cards, TB screening and fingerprint clearance on file.
It is about whether the people giving care have been trained and checked, and whether the home can prove it.
Cited at 46% of similar communities in the past two years.
Ask on a tour How are new caregivers trained, and how long before they work a shift alone?
Admission and residency agreements
What a home must have before a resident moves in, such as a recent health assessment and TB screening, and what the written residency agreement must say.
It is about whether the home knew a resident's needs on day one, and whether the family has the terms in writing.
Cited at 33% of similar communities in the past two years.
Ask on a tour May we read the residency agreement and the move-in assessment before we sign?
Upkeep and hazards
A clean, safe building: chemicals locked away, hot water at a safe temperature, and nothing in the house that could cause a fall or an injury.
These are the everyday safety checks of the place a resident lives.
Cited at 42% of similar communities in the past two years.
Ask on a tour Where are cleaning products and medicines kept?
These explain the rule, not what happened. The state's own report is on AZ Care Check.
How the figures above are counted
- Routine figures: 72 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
- Complaint and enforcement figures: 84 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
- A complaint inspection counts here when it cited at least one deficiency.
- Most ADHS enforcement actions are civil penalties (fines).
- An area is one section of Arizona's assisted living rules, such as Medication or Service plans. A repeat can be a different problem under the same section.
- POC is a plan of correction. IDR is an informal dispute of a citation.
- Counts only: ADHS does not rate or rank homes, and neither do we.

Directed care is the highest of ADHS's three assisted living care levels, meant for people who cannot direct their own care, often because of dementia or another cognitive impairment. The license covers directed care, personal care and supervisory care services.

The company says one all-inclusive monthly fee covers medication management and help with bathing, dressing, mobility and grooming. The website lists chef-prepared meals in a dining room, scheduled transportation, daily exercise classes, and housekeeping and linen service, with activities like bingo, gardening club and casino night. For amenities it names a salon and spa, movie theater, library, coffee bar, fitness center and computer room, plus outdoor gardening space. Pets are welcome, with some breed and weight restrictions.
Ask who works at night and how long the caregivers have been there. You cannot tell that from a photo, so ask in person.
