About Arizona Sunset Assisted Living
ADHS defines directed care as care for residents who cannot direct their own care due to a cognitive impairment. ADHS records list an overall capacity of 10 residents, with directed care service capacity listed at 8, alongside personal care and supervisory care services.
The home's website describes a 6 bedroom, 4 bathroom home that is family owned and operated, with a staff ratio of up to 8 residents to 2 caregivers, usually 6 to 7 residents to two caregivers. It offers both private and semi private rooms, with a private bath option, and prices are set by the level of care required: supervisory, personal or directed care. The website lists resident supervision by a nurse practitioner, medication administration, and help with bathing, dressing and toileting. Caregiving staff rest from 10pm to 6am but remain available for urgent needs overnight, according to the website. Housekeeping includes daily bed making, weekly deep cleaning and laundry at least weekly. The site describes meals prepared daily with fresh ingredients, and activities including music recitals, nature walks, daily movies and painting lessons. The property includes a pool, a piano residents and guests may play, and a nature trail on the 0.83 acre lot with shaded benches and views of the San Tan Mountains. The website says the home is open to visitors of all ages, including children, and that it hosts community volunteer groups; transportation arrangements are available, with additional fees noted by the company.
Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
