Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

No deficiencies were cited at its last routine inspection (July 16, 2026, a visit that also looked into a complaint). 63% of similar communities had at least one cited at theirs.

No deficiencies at either of its last two routine inspections (2025 and 2026).

Three complaint inspections in the past two years led to a citation, and 14 others found nothing to cite. 50% of similar communities had 2 or fewer.

ADHS took four enforcement actions in the past two years, with civil penalties of $2,250 in all. 33% of similar communities had none.

Cited at more than one inspection since December 2023: Upkeep and hazards (six inspections), Administration (five inspections), Emergency and safety (five inspections), and 10 other areas.

ADHS accepted its plan to correct each of the 5 deficiencies cited in the past two years.

Service plans

Each resident's written care plan: what help they get, how often, and who gives it. Caregivers must follow the plan, record what they did, and update it when needs change.

The service plan is the day-to-day care a resident receives. Citations here are about plans missing, out of date, or not followed.

Cited at 50% of similar communities in the past two years.

Ask on a tour How often is the care plan reviewed, and can family take part?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Routine figures: 72 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
  • Complaint and enforcement figures: 84 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans. A repeat can be a different problem under the same section.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

Silver Birch of Midtown is a seven-story assisted living and memory care center at 4000 North 3rd Avenue in Phoenix (Maricopa County). ADHS licenses it as an Assisted Living Center-Directed, the highest of the state's three assisted living care levels, for 165 residents.

Its website describes 140 residences in studio, one-bedroom and two-bedroom layouts, with private and semi-private apartments that each have a private bathroom with a walk-in shower. The company, Silver Birch Living, says its memory care program has dementia-trained staff in a secure, calming environment, and that the community serves residents 55 and older from a range of financial backgrounds under a single monthly fee that varies with each resident's ability to pay. It also lists a sister community, Silver Birch of Avondale.

As the company describes daily life, there are three meals plus snacks in a restaurant-style dining room, transportation to shopping and scheduled outings, 24-hour staff, and housekeeping and laundry service. Under its pet policy, registered service animals are welcome, and caged birds or aquarium fish are allowed with prior written approval; other pets can come only on leashed, temporary visits.

Provider directories for three ALTCS plans, UnitedHealthcare Community Plan, Banner-University Family Care and Mercy Care, have listed this address, some under an earlier name, Bridgewater Assisted Living Midtown. The most recent of those directories is dated July 2026.

Care and services

In our words

ADHS lists the community as licensed for directed care services (capacity 40), personal care services (capacity 125) and supervisory care services. Directed care, the top of the three levels, is for residents who cannot direct their own care, for example because of dementia or other cognitive impairment. The community's website says the memory care program has dementia-trained staff in a secure, calming environment, and the site adds that staff are there 24 hours a day with certified caregivers on site.
