Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

The state cited five deficiencies at its last routine inspection (August 12, 2026, a visit that also looked into a complaint). 55% of similar communities had 2 or fewer at theirs.

The routine inspection before it (2025) cited none.

Two complaint inspections in the past two years led to a citation, and 13 others found nothing to cite. 63% of similar communities had 2 or fewer.

No enforcement action by ADHS in the past two years. The same is true of 44% of similar communities.

Across 23 inspections since December 2023, the state never cited the same area twice.

For the 6 deficiencies cited in the past two years, Care Check lists the plan of correction as: POC Submitted (5), IDR Rejected (1).

What was cited on August 25, 2026, in plain words

Administration

The duties of the owner and manager: written policies, a caregiver in charge on site at all times, background checks, and telling ADHS about changes such as a new manager.

It is about who is accountable for care at any hour and whether the home runs by its own written rules.

Cited at 55% of similar communities in the past two years.

Ask on a tour Who is in charge when the manager is not here, and how do I reach them?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Routine figures: 63 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
  • Complaint and enforcement figures: 76 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans.
  • POC is a plan of correction. IDR is an informal dispute of a citation.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

Encanto Palms Assisted Living is a 97-resident center in Phoenix, and ADHS licenses it at the directed care level, the highest of the state's three assisted living care levels. The license is held by Encanto Palms Healthcare, LLC. The community's website describes three separate units: Sahara, a secured unit for residents with a psychiatric diagnosis; Mirage, a secured memory unit for people with Alzheimer's and other forms of dementia; and Oasis, a non-secured unit for residents whose behaviors don't call for a secured setting. Its privacy practices page places the home within the PACS Group, Inc. affiliated covered entity. The company says psychiatric and medical providers come by weekly, with dental, vision, hearing and podiatry services offered on site, and the site lists a gated perimeter, window alarms and emergency call lights, along with check-ins at least every two hours.

Ask who works at night and how long the caregivers have been there. You cannot tell that from a photo, so ask in person.

Daily life

In our words

The community's website lists plenty to fill a week: bingo, choir, fitness activities, current events discussion, a resident council, veterans and volunteer programs, clubs like a book club and chess club, bible study groups, and outings to movie theaters, stores, restaurants and museums. Its FAQ adds a few house rules. Cell phones aren't allowed in two of the three units, though residents can make calls on the main line, and because of the behavioral setting no visitors under 18 are permitted, while family pets are welcome to visit. A beautician works on site, and residents or families can reserve a common area or the gazebo.
