About Assisted Living at the Dementia Care and Education Campus
Hospice of the Valley operates Assisted Living at the Dementia Care and Education Campus, which opened in 2022 on a 3.2 acre campus in Phoenix's Arcadia neighborhood. The center is licensed for 18 residents at the directed care level, the highest of Arizona's assisted living care levels, and ADHS records also list personal care and supervisory care among its service types. The community's website describes personalized care for people living with all types and stages of dementia, and says doctors, nurses, social workers and dementia educators are always available to meet medication and health needs. Private rooms and a suite are available, and an earlier company release described the assisted living building as housing up to 12 residents with mild, moderate or advanced dementia, some with behavioral problems, who may or may not be on hospice. The campus is designed around a central green space for outdoor time, with a grassy area, planters, benches and walkways, plus separate walking paths with views of Camelback Mountain. A 2021 report on the campus also described a 10 person hospice inpatient care home and a memory cafe on the same site, in addition to the assisted living center. Daily life includes art, music, exercise and cooking activities, along with an onsite chef, housekeeping and salon services.

We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
