Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

The state cited three deficiencies at its last routine inspection (July 17, 2025, a visit that also looked into a complaint). 57% of similar homes had 3 or fewer at theirs.

One complaint inspection in the past two years led to a citation. 60% of similar homes had none.

No enforcement action by ADHS in the past two years. The same is true of 58% of similar homes.

ADHS accepted its plan to correct each of the 3 deficiencies cited in the past two years.

Enforcement actions

AZ Care Check has no enforcement actions on file for this home.

What was cited on July 17, 2025, in plain words

Emergency responder forms

A state law that requires a standard form for each resident, given to paramedics in an emergency: medicines, contacts, and care wishes.

It means paramedics get the facts they need quickly if a resident must go to the hospital.

Cited at 18% of similar homes in the past two years.

Ask on a tour Is the emergency form for each resident kept up to date? Where is it kept?

First aid and CPR

A state law that requires staff to give first aid and start CPR, in line with their training, until paramedics arrive.

It is about what happens in the first minutes of an emergency.

Cited at 1% of similar homes in the past two years.

Ask on a tour Is someone trained in CPR on every shift?

Staffing and staff records

Caregiver qualifications and records: skills checked before working alone, training, CPR and first aid cards, TB screening and fingerprint clearance on file.

It is about whether the people giving care have been trained and checked, and whether the home can prove it.

Cited at 29% of similar homes in the past two years.

Ask on a tour How are new caregivers trained, and how long before they work a shift alone?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Routine figures: 383 licensed assisted living homes with 6 to 10 beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
  • Complaint and enforcement figures: 982 licensed assisted living homes with 6 to 10 beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

ADHS licenses the home for 10 residents at the directed care level, the highest of Arizona's three assisted living care levels. That means it is set up for residents who cannot direct their own care, for example because of dementia or other cognitive impairment.

According to the home's website, a visiting RN sets each resident's care level and drafts a care plan at intake, and a social worker writes a customized care plan for each new resident.

The home's site lists medication management, diabetic care, oxygen support, incontinence care, wound treatment and dementia care, along with coordination with outside agencies such as home health, hospice care and social workers, and on-call doctors, nurses, podiatrists and therapists can be called in. Rooms are private or semi-private with a bath, furnished or unfurnished, and out back there is a patio with a gazebo. Breakfast, lunch and dinner are home cooked and served family style, with snacks in between, fitted to what residents like and with low sodium and low sugar options. Weekly outings round out a calendar of arts and crafts, bingo, board games, cards, music and movie nights.

Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.
