What the inspections show
In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).
The state cited 25 deficiencies at its last routine inspection (December 2, 2025, a visit that also looked into a complaint). 55% of similar communities had 2 or fewer at theirs.
12 deficiencies cited at the routine inspection before it, in 2024.
10 complaint inspections in the past two years led to a citation, and nine others found nothing to cite. 63% of similar communities had 2 or fewer.
ADHS took eight enforcement actions in the past two years, with civil penalties of $9,575 in all. 44% of similar communities had none.
Cited at more than one inspection since March 2024: Administration (eight inspections), Service plans (six inspections), Admission and residency agreements (three inspections), and six other areas.
ADHS accepted its plan of correction for 51 of the 53 deficiencies cited in the past two years. Care Check lists the rest as: POC Submitted (2).

What was cited on June 9, 2026, in plain words
Administration
The duties of the owner and manager: written policies, a caregiver in charge on site at all times, background checks, and telling ADHS about changes such as a new manager.
It is about who is accountable for care at any hour and whether the home runs by its own written rules.
Cited at 55% of similar communities in the past two years.
Ask on a tour Who is in charge when the manager is not here, and how do I reach them?
Emergency and safety
Disaster and evacuation drills, first aid supplies, and what staff must do and record after a resident's accident or injury.
It is about whether staff know what to do in an emergency, and whether the family hears about an injury.
Cited at 39% of similar communities in the past two years.
Ask on a tour When was the last fire drill, and how would you tell me if my parent were hurt?
These explain the rule, not what happened. The state's own report is on AZ Care Check.
How the figures above are counted
- Routine figures: 63 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
- Complaint and enforcement figures: 76 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
- A complaint inspection counts here when it cited at least one deficiency.
- Most ADHS enforcement actions are civil penalties (fines).
- An area is one section of Arizona's assisted living rules, such as Medication or Service plans. A repeat can be a different problem under the same section.
- POC is a plan of correction. IDR is an informal dispute of a citation.
- Counts only: ADHS does not rate or rank homes, and neither do we.

Overview
In our words
Directed care is the highest of the state's three assisted living tiers, and it is meant for residents who can no longer direct their own care, for example because of dementia or other cognitive impairment.
Mosaic Management, the company that runs the home from Salem, OR, describes it as a memory care community for residents with early to late stage dementia and Alzheimer's, with trained caregivers on hand 24/7 and a secured perimeter with secured entrances and exits. Its site lists two floor plans, a studio of about 241 square feet and a companion suite of about 290 square feet. Residents get reminders and help at mealtimes, help with their personal laundry and a personal emergency response system in every residence, and the hallways and bathrooms have handrails and grab bars. The company also lists local transportation, religious services on site, and a calendar of educational events, social gatherings, hobby clubs, fitness classes and movie nights.
Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.
