Highlights
- ADHS licenses Harvest at Queen Creek Assisted Living for 10 residents at the directed care level, the highest of Arizona's three assisted living care levels.

Care and services
Harvest at Queen Creek Assisted Living holds an ADHS license for directed care, which is the level for residents who cannot direct their own care, for example because of dementia or another cognitive impairment. AZ Care Check breaks that down further: directed care services for up to 8 residents, alongside personal care services and supervisory care services, all within the home's capacity of 10. ADHS does not license a distinct memory care designation, so memory care is the home's own term rather than a state license category. Its website describes memory care programs designed for people living with dementia and Alzheimer's.
The neighborhood
For errands, the nearest pharmacy is a Walgreens about 1.49 miles away, and the nearest grocery store is a Fry's Marketplace about 1.7 miles away. Two more hospitals, East Valley ER & Hospital in Gilbert and Arizona General Hospital in Mesa, are within 10 miles.
The company
ADHS records show the license held by Harvest at Queen Creek Assisted Living LLC, and the home goes by that same name, with a website at harvestalqc.com. A September 2024 post on its Nextdoor business page said the home was ready for occupancy then.

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
