How the figures above are counted

  • Routine figures: 81 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas whose last routine inspection was between September 2024 and September 2026.
  • Complaint and enforcement figures: 76 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans. A repeat can be a different problem under the same section.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

Countryside Living is the trade name for COUNTRYSIDE SENIOR LIVING, an assisted living center at 22773 East Ocotillo Road in Queen Creek (Maricopa County). The license is held by COUNTRYSIDE AT OCOTILLO, LLC, and the community's website names American Care Concepts as the company behind it.

According to the community's website, staff handle medication administration and management, incontinence care and help in the restroom, and they coordinate with outside providers, hospice included. Licensed nurses are onsite and complete the required nursing assessments and service plans for each resident, and caregivers are there through the night. There is also a memory care program, with staff specially trained for memory support, wander management technology, and a secured setting that covers the indoor common areas and an outdoor courtyard.

The website also describes a building with no resident stairwells, elevators or long hallways, with outdoor porches, walking paths, a salon and equine therapy among its amenities. Units come in five floor plans from 344 to 515 square feet, with private bathrooms. Culinary chefs cook with fresh, locally sourced ingredients, and dining runs around the clock, outdoors as well as in. The activity list takes in music therapy, live musical events, card games, group outings, community events and pet therapy, transportation goes to outside appointments and events, and the community is pet friendly.
