About Mountain Park Senior Living
AZ Care Check divides that capacity by service type: 20 directed care, 66 personal care, and supervisory care.
The community is operated by Spectrum Retirement Communities, LLC, headquartered in Denver, Colorado. The community's website lists it alongside eight other Arizona communities: The Enclave at Gilbert Senior Living, The Enclave at Chandler Senior Living, The Enclave at Anthem Senior Living, Palos Verdes Senior Living, Lone Mountain Memory Care, The Pavilion at Camelback Senior Living, Las Palomas Senior Living and Gardens at Ocotillo Senior Living. A 2012 article in the Ahwatukee Foothills News reported that the community opened that year with 138 apartments, 60 for assisted living and 18 for memory care, ranging from 398 to 1,200 square feet.
The community's website describes a Montessori-based memory care program using the St. Louis University Mental Status Exam (SLUMS) for cognitive screening. The site also says it does not offer skilled nursing or rehabilitation, and that residents are on a month-to-month rental arrangement.
Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
