About Everlasting Services Estrella Center
The company, Everlasting Services, says it was established in 2006 and operates three locations, including this 124-bed community, which it calls its flagship and its Estrella Center.
The community's website says the center specializes in brain injury and memory care and includes a special care unit (SCU) for residents who require a higher level of care. It describes the building as secured to keep residents who may become confused or wander safe, with 24/7 awake staff. The site also describes apartment-style rooms with large closets and zero-barrier showers, a large courtyard with a walking path, an on-site gym for resident use or neuro therapies, and an educational pavilion that held a ribbon cutting ceremony on January 31, 2025, funded in part by an AHCCCS grant using American Rescue Plan funds.
The community's website also says the center accepts all ALTCS plans, workers' compensation, and private pay. Separately, provider directories list the facility in the ALTCS networks of UnitedHealthcare Community Plan (list dated July 28, 2025) and Banner-University Family Care (list dated July 2026).

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
