Overview
In our words
Bella Vita at Cholla is a 10 resident assisted living home at 4511 East Cholla Street in Phoenix (Maricopa County). ADHS licenses it at the directed care level, the highest of Arizona's three assisted living care levels, which is meant for residents who can't direct their own care, such as people with dementia or other cognitive impairment.
ADHS lists directed, personal and supervisory care services for the home, and its website lists the same. The site adds that every resident has a private bedroom with a private bathroom, staff are awake overnight, and medication management, incontinence care and hospice services are offered so residents can age in place. Physical, occupational and speech therapists visit, x-ray and lab work is done onsite, and services may also include visiting physicians, podiatrists and geriatric psychiatrists. It also says staff take dementia certification training every year, and that Bella Vita is a member of the Parkinson's Foundation's Community Partners in Parkinson's Care, with staff trained to handle subcutaneous pumps for residents with Parkinson's disease.
Outside, the company describes a backyard with a walking path, a gated and alarmed pool, a vegetable garden and a covered gazebo, with keypad locks and alarms on the entry and exit doors. Meals come three times a day with unlimited snacks, and special diet requests can be accommodated.
Bella Vita at Cholla is the trade name the home goes by on its website, which also describes two other Bella Vita properties in Arizona.
