The state cited four deficiencies at its last routine inspection (March 4, 2026). 60% of similar communities had 2 or fewer at theirs.

No complaint inspections since November 2024.

AZ Care Check lists no enforcement action against this license.

Across three inspections since November 2024, the state never cited the same area twice.

ADHS accepted its plan to correct each of the 4 deficiencies cited since November 2024.

Enforcement actions

AZ Care Check has no enforcement actions on file for this home.

What was cited on March 4, 2026, in plain words

Emergency responder forms

A state law that requires a standard form for each resident, given to paramedics in an emergency: medicines, contacts, and care wishes.

It means paramedics get the facts they need quickly if a resident must go to the hospital.

Cited at 28% of similar communities in the past two years.

Ask on a tour Is the emergency form for each resident kept up to date? Where is it kept?

Staffing and staff records

Caregiver qualifications and records: skills checked before working alone, training, CPR and first aid cards, TB screening and fingerprint clearance on file.

It is about whether the people giving care have been trained and checked, and whether the home can prove it.

Cited at 44% of similar communities in the past two years.

Ask on a tour How are new caregivers trained, and how long before they work a shift alone?

Upkeep and hazards

A clean, safe building: chemicals locked away, hot water at a safe temperature, and nothing in the house that could cause a fall or an injury.

These are the everyday safety checks of the place a resident lives.

Cited at 34% of similar communities in the past two years.

Ask on a tour Where are cleaning products and medicines kept?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Routine figures: 81 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas whose last routine inspection was between September 2024 and September 2026.
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

Spring Gardens Peoria is an assisted living center for 94 residents at 26850 North Lake Pleasant Parkway in Peoria (Maricopa County, Arizona). ADHS licenses it for directed care (24 residents), personal care (70 residents) and supervisory care, the three assisted living care levels the state recognizes. Directed care is the highest of them and is meant for residents who cannot direct their own care because of dementia or another cognitive impairment. According to the community's website, the property is run by Avista Senior Living, a company headquartered in Mesa, Arizona that calls itself a name in senior living since 2011, and this location is now under new management. The memory care program it describes combines exercise, nutrition, medication management and daily activity. Suites come in studio, one bedroom and two bedroom layouts, each with a private bathroom and full shower, and kitchenettes are listed among the unit features. Dining is restaurant style, housekeeping and laundry come weekly, and there is transportation to scheduled doctor's appointments. For the rest of the week there are community outings, exercise classes, an art studio and a happy hour, plus a full service spa with hairstyling and a nail bar. The building has a 24 hour emergency call and response system, a backup generator and a sprinkler system throughout, with courtyards for time outdoors.
