Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

The state cited one deficiency at its last routine inspection (February 24, 2026, a visit that also looked into a complaint). 55% of similar communities had 1 or fewer at theirs.

2 deficiencies cited at the routine inspection before it, in 2025.

Two complaint inspections in the past two years led to a citation, and one other found nothing to cite. 50% of similar communities had 2 or fewer.

No enforcement action by ADHS in the past two years. The same is true of 33% of similar communities.

Across six inspections since December 2023, the state never cited the same area twice.

ADHS accepted its plan to correct each of the 3 deficiencies cited in the past two years.

Staffing and staff records

Caregiver qualifications and records: skills checked before working alone, training, CPR and first aid cards, TB screening and fingerprint clearance on file.

It is about whether the people giving care have been trained and checked, and whether the home can prove it.

Cited at 46% of similar communities in the past two years.

Ask on a tour How are new caregivers trained, and how long before they work a shift alone?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Routine figures: 72 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
  • Complaint and enforcement figures: 84 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

The center holds a directed care license, the highest of Arizona's three assisted living designations, and ADHS records list all three service types at the facility (directed, personal and supervisory care), with a licensed capacity of 121 residents.

MBK Senior Living has run the community since November 2021, when it bought the property from its previous company and changed the name from Mariposa Point of Mesa to Sky Vista. MBK's announcement of the purchase describes one 62,000 square foot, single story building with 83 units, and says the deal brought MBK to five communities in the Valley, out of 34 nationwide at the time.

According to the community's website, the memory care neighborhood is called Connections for Living and has its own secured courtyard, where music, art and pet therapy are offered from time to time through the MBK Mind + Body Wellness Memory Care program. Assisted living apartments come in studio, one bedroom and two bedroom floor plans with private rooms, each with a kitchenette holding a refrigerator and microwave, while memory care has semi-private or shared suites. The site also lists in-room dining, a full service salon and spa, a fitness center and an emergency alert system in every home, and says utilities are included except telephone. Assisted living caregivers are on hand around the clock and short term stays are offered. Meals are served table style, with anytime dining from 7:00 a.m. to 7:00 p.m. daily, and the property has views of the Superstition Mountains.

MBK Senior Living is based in Irvine, CA, and is a subsidiary of MBK Real Estate LLC. Buying this community gave the company five in Arizona, out of 34 nationwide at the time.
