Arizona state record
What the inspections show
In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).
The state cited three deficiencies at its last routine inspection (July 29, 2026, a visit that also looked into a complaint). 55% of similar communities had 1 or fewer at theirs.
Three complaint inspections since October 2025 led to a citation, and four others found nothing to cite.
AZ Care Check lists no enforcement action against this license.
Across eight inspections since October 2025, the state never cited the same area twice.
ADHS accepted its plan to correct each of the 6 deficiencies cited since October 2025.

Enforcement actions
AZ Care Check has no enforcement actions on file for this home.
What was cited on July 29, 2026, in plain words
Staffing and staff records
Caregiver qualifications and records: skills checked before working alone, training, CPR and first aid cards, TB screening and fingerprint clearance on file.
It is about whether the people giving care have been trained and checked, and whether the home can prove it.
Cited at 46% of similar communities in the past two years.
Ask on a tour How are new caregivers trained, and how long before they work a shift alone?
Directed care services
Care for residents who cannot direct their own care, such as many people with dementia: closer care planning, weight checks, and exits that are safe for them.
These rules protect residents who cannot speak up for themselves.
Cited at 26% of similar communities in the past two years.
Ask on a tour How do you keep residents with dementia safe without locking them in?
These explain the rule, not what happened. The state's own report is on AZ Care Check.
How the figures above are counted
- Routine figures: 72 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
- An area is one section of Arizona's assisted living rules, such as Medication or Service plans.
- Counts only: ADHS does not rate or rank homes, and neither do we.

Overview
In our words
State records list three levels of care here: supervisory, personal and directed. Pegasus Senior Living has run the community since 2018 and took over the property itself in December 2024, when the name changed from Broadway Mesa Village to Pegasus Landing of Mesa. The community's website describes a memory care program called Connections, staffed with a licensed nurse and caregivers trained to support residents with dementia or Alzheimer's. It also lists medication management, incontinence care, diabetic care, respite stays, physical, occupational and speech therapy on site, and support 24 hours a day.
Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.
