About Heritage Lane Behavioral - West
Heritage Lane Behavioral - West operates as an assisted living center in Mesa, licensed by ADHS at the personal care level, which covers help with activities of daily living such as bathing, dressing, grooming and toileting, plus medication administration.
According to the community's website, the center is built as two single story buildings with 57 private suites, and it specializes in caring for residents whose behavioral issues are related to specific mental health diagnoses. The same website states that admission criteria exclude a primary diagnosis of dementia or Alzheimer's disease and a recent history of drug or alcohol abuse, since the program requires residents to be able to participate in their own care planning.
The company also says all employees complete a minimum of 10 hours of behavior training each year, that caregivers are not assigned other duties such as cooking, and that the center does not use agency or registry staff. Meals are cooked from raw ingredients rather than pre-processed foods, and a weekly resident Food Council votes on the menu. Activities run seven days a week until as late as 7:00 pm and include morning movement sessions designed by a licensed physical therapist, evening activities, three weekly community outings and a yearly Bucket List Activity.
The company says the center accepts private pay, and it is listed in the ALTCS networks of: UnitedHealthcare Community Plan (ALTCS) (list dated July 28, 2025).

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
