About Extended Family Home - West
Extended Family Home - West is one of 2 residential homes that Extended Family Homes has run in Mesa since 1996, according to the home's website. ADHS licenses the home for 10 residents at the directed care level, the highest of the state's assisted living care levels, meaning it can serve residents who cannot direct their own care, including those with dementia. The home's website describes the home as providing personalized memory care for adults with Alzheimer's disease and other forms of dementia, and says its doors and windows are equipped with active alarms to alert staff to unexpected entry or exit. Staff receive eight hours of memory care training during orientation and four hours of continuing education each year, per the company.
Caregivers help with bathing, dressing, meals and mobility, and assist with medication reminders and administration, the home's website says. It also describes access to in-house physicians, RN on-call support, hospice support and physical therapy services. The home offers both private and semi-private rooms, with at least one fully ADA-accessible shower, and the company says residents have use of large backyards and covered patios. Meals are described as prepared fresh daily and served family-style, with weekly laundry service and regular room cleaning. The home's website also lists recreational activities, social engagement, Smart TV access, Wi-Fi and grooming services such as manicures, pedicures and haircuts. The company describes its pricing as all-inclusive and says a private long-term care insurance policy may help cover costs.

Our referral disclosure
Before we refer you to any assisted living home, we send you this notice to read and sign. Here it is now, so you can read it at your own pace.
Arizona law requires that we provide you with the following disclosure notice.
We are in the business of referring residents to assisted living facilities and assisted living homes. We will be paid by the assisted living facility or assisted living home if you move into one of the referred assisted living facilities or assisted living homes. The fee we receive from the assisted living facility or assisted living home into which you move typically ranges from 70 to 100 percent of your first month's rent and care charges. We do have a current business relationship but we do not have a common ownership or control in, or any other financial, business, management or familial relationship with, any of the assisted living homes and assisted living facilities to which we are referring you.
By providing us with a written or electronic notice, you have the right to terminate our services to you at any time, including our use of your personal information. If you terminate our services, we will not be entitled to any fee for any move-in you make after the date of the termination notice unless either:
1. The assisted living facility or assisted living home you choose within the next twelve months is one that we specifically identify and refer to you after we evaluate your profile and requests but before we receive your notice of termination.
2. You communicate with us before you move into the assisted living facility or assisted living home.
To end our services at any time, reply to any email from us or call Integrity Senior Placement at 480.271.7759.
