Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

The state cited two deficiencies at its last routine inspection (December 31, 2025, a visit that also looked into a complaint). 61% of similar communities had 2 or fewer at theirs.

7 deficiencies cited at the routine inspection before it, in 2024.

Two complaint inspections in the past two years led to a citation, and four others found nothing to cite. 62% of similar communities had 2 or fewer.

ADHS took one enforcement action in the past two years, with civil penalties of $500 in all. 45% of similar communities had none.

Cited at more than one inspection since June 2024: Directed care services (two inspections).

ADHS accepted its plan to correct each of the 9 deficiencies cited in the past two years.

What was cited on December 31, 2025, in plain words

Directed care services

Care for residents who cannot direct their own care, such as many people with dementia: closer care planning, weight checks, and exits that are safe for them.

These rules protect residents who cannot speak up for themselves.

Cited at 25% of similar communities in the past two years.

Ask on a tour How do you keep residents with dementia safe without locking them in?

Medication

How medicines are stored (locked, at the right temperature), given exactly as the doctor ordered, and recorded each time.

A missed, late or wrong dose can change how a resident feels and how safe they are.

Cited at 37% of similar communities in the past two years.

Ask on a tour Who gives medicines here, how were they trained, and how is each dose checked?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Routine figures: 159 licensed assisted living centers (11 or more beds) in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
  • Complaint and enforcement figures: 198 licensed assisted living centers (11 or more beds) in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans. A repeat can be a different problem under the same section.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

ADHS lists Sarah's Place at Glencroft as an assisted living center at 6670 West Butler Drive in Glendale, Maricopa County.

According to a 2012 news article, the new home sat within the Glencroft retirement community, in two buildings with 12 individual rooms each (24 in all), and every room had its own bathroom. The two buildings shared a salon, and each had a large kitchen and a common area with a flat screen TV, a fireplace and large bookcases.

The home is also on the ALTCS network lists of two plans, which appear under rooms and pricing.

According to the 2012 news article, the home is named for Sarah Ruth, who gave the seed money to build the Glencroft retirement community. Glencroft is described there as a not-for-profit community under the sponsorship of Mennonite Health Services Alliance, with residential apartments, rentals, HUD housing, assisted living, and a nursing home and rehab center on the same campus, and Sarah's Place was its first new green construction project. That picture is from 2012, so parts of it may have changed.

Visit in the afternoon and sit in the dining room for a while. Ask to see the actual room, not the model.

Daily life

In our words

The 2012 news article came back to the shared spaces more than once. Residents could get their hair cut and styled at the salon the two buildings have in common, while each building had its own big kitchen and a common area with a flat screen TV, a fireplace and large bookcases. Residents were also encouraged to bring things from home so their rooms would feel more comfortable.
