Arizona state record
What the inspections show
In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).
No deficiencies were cited at its last routine inspection (October 7, 2025, a visit that also looked into a complaint). 63% of similar communities had at least one cited at theirs.
8 deficiencies cited at the routine inspection before it, in 2024.
One complaint inspection in the past two years led to a citation, and one other found nothing to cite. 50% of similar communities had 2 or fewer.
No enforcement action by ADHS in the past two years. The same is true of 33% of similar communities.
Cited at more than one inspection since December 2023: Administration (two inspections).
ADHS accepted its plan to correct the deficiency cited in the past two years.

Emergency responder forms
A state law that requires a standard form for each resident, given to paramedics in an emergency: medicines, contacts, and care wishes.
It means paramedics get the facts they need quickly if a resident must go to the hospital.
Cited at 26% of similar communities in the past two years.
Ask on a tour Is the emergency form for each resident kept up to date? Where is it kept?
These explain the rule, not what happened. The state's own report is on AZ Care Check.
How the figures above are counted
- Routine figures: 72 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
- Complaint and enforcement figures: 84 licensed assisted living centers with 100 or more beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
- A complaint inspection counts here when it cited at least one deficiency.
- Most ADHS enforcement actions are civil penalties (fines).
- An area is one section of Arizona's assisted living rules, such as Medication or Service plans. A repeat can be a different problem under the same section.
- Counts only: ADHS does not rate or rank homes, and neither do we.

Overview
In our words
Sky Ridge holds an ADHS license as an Assisted Living Center-Directed, for 121 residents at the directed care level. That is the highest of the three ADHS assisted living care levels, meant for residents who cannot direct their own care, for example because of dementia or other cognitive impairment. Its listed services are directed care, personal care and supervisory care.
According to its website, Sky Ridge is run by MBK Senior Living, a company based in Irvine, California, with 35 communities across the western United States, including a sister community in Gilbert, Savanna House. The community used to be Mariposa Point of Gilbert and became Sky Ridge after MBK Senior Living bought it in February 2022. It has 83 units in studio, one bedroom and two bedroom floor plans, with private and shared suites that have kitchenettes. The building is single-story, set around an expansive courtyard with landscaping and walking paths.
The website also describes a memory care program, the MBK Mind + Body Wellness Memory Care program, for residents with Alzheimer's disease and other forms of dementia, with music therapy, art therapy and pet therapy offered from time to time and caregivers on hand to respond to residents' needs around the clock. Beyond that it lists restaurant-style anytime dining from 7:00 a.m. to 7:00 p.m. daily, meals in the room, group outings, transportation, a salon and spa, and covered parking. Pets are welcome and the community is smoke-free.
