Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

Licensed in September 2012. AZ Care Check lists no licensing inspection for it. It has not had a routine inspection yet. Among the homes on file, a new home's first routine inspection has come a median of 17 months after its licensing inspection.

One complaint inspection in the past two years led to a citation. 60% of similar homes had none.

No enforcement action by ADHS in the past two years. The same is true of 58% of similar homes.

ADHS accepted its plan to correct the deficiency cited in the past two years.

Emergency responder forms

A state law that requires a standard form for each resident, given to paramedics in an emergency: medicines, contacts, and care wishes.

It means paramedics get the facts they need quickly if a resident must go to the hospital.

Cited at 18% of similar homes in the past two years.

Ask on a tour Is the emergency form for each resident kept up to date? Where is it kept?

These explain the rule, not what happened. The state's own report is on AZ Care Check.

How the figures above are counted

  • Complaint and enforcement figures: 982 licensed assisted living homes with 6 to 10 beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

ADHS licenses the home for up to 10 residents at the directed care level, the highest of the three assisted living care levels the state defines, and its facility record lists directed, personal and supervisory care services as approved. The home's own website describes the care another way, as help with daily activities such as bathing, grooming, dressing, feeding and medication administration, all given under a physician's signed orders.

The company says it has provided care since 2002 and runs four locations in the Gilbert and Queen Creek area.

According to its website, the home can arrange a visiting physician, a visiting podiatrist, home health therapists and hospice care in place, and a registered nurse prepares the service plan. Caregivers can handle Hoyer lift transfers, peg tube feeding and insulin-dependent diabetic care, and a pharmacy delivers prescriptions to the home. Certified, fingerprinted caregivers supervise 24 hours a day, and the day brings three meals plus two snacks, individual and group activities and transportation to doctor's appointments, along with wifi, cable television, a daily newspaper subscription, laundry service, mobile dentistry and home hair care visits.

On payment, the home's site says it takes private pay, long-term care insurance and Medicaid through United Healthcare, though only at selected locations. Apart from the website, the home is listed in the ALTCS networks of UnitedHealthcare Community Plan (list dated July 28, 2025).
