Arizona state record

What the inspections show

In Arizona, the Department of Health Services licenses assisted living homes and communities, inspects them and looks into complaints. Its record for each one is public on AZ Care Check (this home's record, opens in a new tab).

The state cited one deficiency at its last routine inspection (July 20, 2026, a visit that also looked into a complaint). 53% of similar communities had more cited at theirs.

The routine inspection before it (2025) cited none.

Two complaint inspections in the past two years led to a citation, and five others found nothing to cite. 63% of similar communities had 2 or fewer.

No enforcement action by ADHS in the past two years. The same is true of 44% of similar communities.

Across eight inspections since December 2023, the state never cited the same area twice.

ADHS accepted its plan to correct each of the 4 deficiencies cited in the past two years.

How the figures above are counted

  • Routine figures: 63 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas whose last routine inspection, between September 2024 and September 2026, also looked into a complaint.
  • Complaint and enforcement figures: 76 licensed assisted living centers with 50 to 99 beds in the Phoenix and Tucson areas licensed for the whole period, September 2024 to September 2026.
  • A complaint inspection counts here when it cited at least one deficiency.
  • Most ADHS enforcement actions are civil penalties (fines).
  • An area is one section of Arizona's assisted living rules, such as Medication or Service plans.
  • Counts only: ADHS does not rate or rank homes, and neither do we.

Overview

In our words

The Enclave at Anthem Senior Living is at 42015 North Venture Drive in Anthem, and the Arizona Department of Health Services licenses it as an assisted living center for 90 residents. ADHS records split that number by the kind of help given: directed care for up to 26 residents, personal care for up to 64, and supervisory care services as well. The subtype on file is Assisted Living Center-Directed, the highest of the three ADHS assisted living care levels, so the community is licensed to care for residents who can no longer direct their own care, including people with dementia or other cognitive impairment.

Spectrum Retirement Communities, a company based in Denver, Colorado, runs the community. Its own site for the property describes a Montessori-based approach to memory care and says the care team uses the St. Louis University Mental Status Exam (SLUMS) as a cognitive screening tool.

According to a 2017 opening announcement, the property was built on 6.5 acres with about 160,000 square feet of living space and 159 apartments, from studios up to two bedroom units. The release placed it near Anthem Community Park, in what it called the North Black Canyon Corridor.
